When we receive a referral for a placement in our homes, we will arrange to carry out an assessment of the person involved.
This includes spending time with the person and also talking to other people involved in their life and care, including family, their current care provider, if appropriate, advocates and the person’s care manager.
The process is designed to establish that our services are best suited to the individual’s needs and to make sure they are comfortable with joining us.
Following the assessment, if we believe that our home is the appropriate place for the individual, we will provide a placement offer, including details of costs. We will also arrange for the person joining us and their family, carer or advocate to visit the home, as part of a transition process that will involve them meeting and starting to get to know the other residents and, for example, joining them for a meal and staying overnight.
During their settling in period, we will carry out a review with the resident and their family, carer or advocate to ensure that we are delivering the right care package to match the person’s needs and goals.
